The municipality of Istočni Stari Grad (Источни Стари Град, East Stari Grad) was formed in the 1990s from parts of the previous municipality of Stari Grad. Initially named Srpski Stari Grad, the municipality was renamed Istočni Stari Grad in 2004.
The seat of administration is in Hreša.

The coat of arms of Istočni Stari Grad (municipal website) is a white shield quartered by a cross formed of two horizontal red over blue stripes and of two vertical red and blue stripes, the chief divided per fess by a red and blue bar. In the first field a golden Latin cross with four squares tightly around it, in the second a gray masoned tower, in the third a black hammer hitting a pile of gray rocks and in the fourth a green bunch of coniferous trees voided. Over the intersection of the stripes the emblem of Republika Srpska proper and in the chief inscribed the name of municipality.
Before 2007, the same coat of arms was used with the coat of arms of Republika Srpska in the inescutcheon. Until 2004, the inscription was probably "Srpski Stari Grad".
