The Military and Hospitaller Order of Saint Lazarus of Jerusalem in Canada has had its arms registered on 1991-05-09: http://reg.gg.ca/heraldry/pub-reg/project.asp?lang=e&ProjectID=1504&ShowAll=1 with the following blazon: Argent a cross Vert and in the first quarter a maple leaf Gules.
The flag, which is the banner of arms, was granted on 1998-04-15:

While no other image of the flag can currently be found on the Web, the badge,
which was granted along with the flag (see URL above), is indeed used at the
website of the Order's Grand Priory in Canada:
http://www.stlazarus.ca, so the flag is probably used as well, although the
details of the design in actual use are yet to be determined.
