quoting from www.heritage.perm.ru/gerbs/gerbs000.htm

Flag adopted on 10 June 1999. The contemporary flag of Perm is rectangular of red color, with the double-sided image in the center of the flag of the basic elements of the coat of arms of Perm city — white polar bear going to the left (to the hoist), carrying on his back a yellow gospel, above it a white cross with the expanding ends. The reverse side of flag is the mirror image of its face side (bear goes to the left from the viewer). Ratio 2:3

Distinction of emblems of the Perm Territory and the Perm City: The cross is lifted above the gospel on the city coat of arms, but the cross lays on the gospel on the regional coat of arms. The princely cap crowns the shield on the regional coat of arms.
